• To avoid scheduling conflicts please complete this form and submit it two weeks prior to your equipment request date.
• Only Faculty members may reserve portable equipment.
• Requests for portable equipment will be confirmed as soon as possible by email.
• Reservations are scheduled on a first come-first serve basis.
• Students who wish to reserve equipment must contact AV services or get a reservation through a professor.
