Program Application Instructions For
the Department of Communication Sciences and Disorders
- Submit a Personal Statement.
- This consists of a two to three page double-spaced word-processed
statement including:
- reason for pursuing graduate study;
- self-evaluation of academic abilities;
- career goals, and
- plan for completion of graduate school.
- Letters of Recommendation and Rating Forms. Note your category
below.
- Bachelor's Degree in Communicative Disorders NOT from UW-Whitewater
- One (1) letter of recommendation and the rating
form from an academic instructor in Communicative Disorders.
- One (1) letter of recommendation and the rating
from a clinical supervisor if applicable. If no clinical
experience, submit a second academic recommendation and
rating form.
- Bachelor's Degree in Communicative Disorders from UW-Whitewater
- One (1) letter from an academic instructor outside
of the Department of Communicative Disorders.
- Submit an exemplary project for faculty review
from your undergraduate portfolio. You will be rated by
the faculty on your academic performance, potential for
clinical development, interpersonal skills and ethical
behavior.

- Deadlines and Credits.
- All applicants are responsible for ensuring a complete application.
You will not be notified of missing documents. All graduate
students begin the program in the Fall semester with the expectation
of a full time credit load of 12 credits. Students with exceptional
needs are accommodated on an individualized basis.
