GUIDE SHEET FOR EVALUATION
OBSERVATION/PARTICIPATION STUDENTS
University of Wisconsin-Whitewater
TO THE COOPERATING TEACHER:
Please consider the following items as you complete this
evaluation form for your University of Wisconsin-Whitewater
Observation and Participation student:
UW-Whitewater students are in their first experience
as a teaching participant in the teaching/learning process.
The students have not had any methods course instruction
and are not expected to be prepared to make complete,
original lesson plans.
The limited time frame/number of days on site will not
make an in-depth evaluation possible.
The evaluation items are related to the activities listed
on page 15 of the Pre-Clinical Program Handbook. The suggested
activities can be adjusted to meet the needs of the cooperating
teacher, the students in the classroom, and the university
student.
This form is one of the set of evaluation forms that
the Department of Public Instruction requires be kept
on file as support for granting an individual a teaching
certificate.
Fifty percent of a passing grade in the Observation and
Participation portion of the Pre-Professional Block Program
is based on this evaluation. The overall score average
of the items marked must be a minimum of 2. If you feel
the university student is likely to receive an overall
score average below 2, you should share this information
with the student and the university coordinator for your
building. This information should be communicated during
the fourth or fifth week of the field experience so that
the university student has an opportunity to make appropriate
modifications.
Please indicate your perception of the student's progress
by circling the appropriate dot on the continuum or the NA
(Not Applicable) given on the evaluation sheet. Write a narrative
statement noting the student's demonstrated strengths in relation
to future success in teacher training.
When completed, this form should be shared with and given
to the student to submit to the university coordinator.
After reviewing and recording the scores on these forms, the
university coordinator will deliver them to the Office of
Field Experiences where they will remain on file. Thank you
for your contribution in this initial step in the preparation
and induction of teachers.
